Its currently an issue with XBox in general. XBox Support is aware of the problem and is working on a fix. This is happening across all games on the Xbox One.Xbox 360 (from an actual 360 console, not a BC game on the X1) and PC seem to be unaffected from my personal experience and from what others have said.
UPDATE: Xbox Achievements aren't unlocking for a lot of players
